Community/Facility info: Federally Qualified Health Center. Our goal is to deliver excellent diagnostic treatment & support services; educate & promote lifestyle changes to eliminate health care disparities & improve the quality of life in the communities of our county. Serves over 7,000 patients annually, providing over 23,000 face-to-face patient visits. The Chief Executive Officer and management staff are highly credentialed community leaders. All clinical staff is board-eligible or board-certified. The patients that receive services are of multi-cultures. Eighty percent of patients have incomes below the 200 percent federal poverty level. More than half of the patients served are uninsured. Centers receive a large portion of their income from the federal government for assisting in providing services to the uninsured and underinsured. Centers offer services on a sliding scale with minimum payment scale for those under 100 percent federal poverty level. Centers are reliant on Medicaid, Medicare, private insurance, patient fees, donations, and local grants for financial assistance.
