Description: Multidisciplinary Heart & Vascular Program is seeking a fellowship trained Advanced Heart Failure Cardiologist
Must be BC/BE in Cardiology and Advanced Heart Failure Cardiology
Contemporary experience with management of patients on mechanical circulatory support
Functions appropriately as part of care delivery team through efficient use of resources and skills, both their own and those of other provider and staff
Actively participates in multi-disciplinary team approach (oversight of advanced practitioners)
Responsibilities include continued development of the current Heart Failure Clinics to improve access and extend the scope of services to identify patients at risk for mortality, hospital readmission, and other adverse events to ensure appropriate evidence based medical and device therapies are provided
Function as a collaborative physician team member within the comprehensive inpatient Heart Failure service including consultative, diagnostic and treatment for high risk hospitalized patients
Identification and evaluation of patients for LVAD candidacy including destination therapy and bridge to transplantation along with coordination of care with neighboring transplantation centers as appropriate
Full complement Cardiovascular Service line (Electrophysiology, Structural heart with complex interventional cardiology, Advanced imaging, Cardio-Thoracic Surgery and Vascular Surgery)
Involved in both outpatient and inpatient AHF responsibilities to include consultative with strong primary hospitalist service support
Full range heart failure program, including:
• Temporary mechanical circulatory support, including IABPs, Impella, RVADs, ECMO.
• Dedicated heart failure treatment unit with trained AHF nurses and excellent NP/PA support
• Robust Durable LVAD program: HM2 and HM3, with myocardial recovery protocol in place
• Heart transplantation (via our collaborating site)
• Cardio-MEMS remote monitoring
• HeartLogic remote monitoring
• Heart failure support and education program with dedicated nurse educators, a Disease management clinic and dedicated heart failure pharmacist
• Cardiac MRI, PET, PYP scan and cCTA with CT FFR, one of a handful in the state
• Cardiac amyloid and Cardiomyopathy programs
• Hypertrophic cardiomyopathy program
• Pulmonary hypertension program
• Cardio-genetics program
• Cardio-obstetrics program
• Heart Failure prevention program
• Mitra-Clip and TAVR
• Cardio-pulmonary exercise test (CPET)
• Right heart catheterization and endomyocardial biopsy
• Growing portfolio of Heart Failure Clinical research
• Cardiac contractility modulation (CCM)
• Diuresis clinic
• Atrial and ventricular arrythmia ablation program
