Description: Facility is seeking Family Medicine Nurse Practitioner to join team
- Examines and treats patients to ensure continuity of care.
- Conducts comprehensive patient assessments, including presenting problems, medical history, allergies, psychosocial factors, medications, and living arrangements.
- Reviews abnormal lab values and determines appropriate treatment when the primary physician is unavailable.
- Identifies needs for patient and family teaching or referrals and prioritizes needs based on assessment and patient concerns.
- Develops teaching plans based on mutually established goals and documented patient needs.
- Involves family members or significant others in the care plan when appropriate.
-Educates patients and families about wellness, illness, and rehabilitation, explaining procedures clearly and sensitively.
- Documents all patient education, including comprehension, response, and follow‑up needs.
- Participates in orientation and teaching of new employees or students.
- Develops and maintains systems for patient education materials.
- Performs physical exams and preventive health measures within physician‑approved protocols.
- Orders, interprets, and evaluates diagnostic tests to assess clinical problems and healthcare needs.
- Records physical findings and formulates care plans and prognoses based on patient condition.
- Submits patient care plans and goals for periodic physician review.
- Prescribes or recommends medications and therapeutic treatments within guidelines.
- Refers patients to physicians or specialized resources when appropriate.
- Assumes additional duties consistent with the role.
- Participates in supervision activities.
- Takes part in an on‑call rotation to provide prompt support for urgent patient needs
- Required: FNP Licensed, Current Massachusetts Nurse Practitioner License Board Certification, DEA, CPR/BLS certificate
