Description: Hospital with 2 satellite clinics is seeking a BC/BE Primary Care Physician - BC/BE Internal Medicine, Family Medicine, or Med/Peds required - Joint Commission accredited facility serving a diverse underserved and general community population - Conduct thorough patient evaluations including medical histories, physical examinations, and diagnostic test ordering - Develop and implement individualized treatment plans; manage chronic conditions including diabetes, hypertension, and asthma - Provide preventive care including routine check-ups, immunizations, and health screenings; address acute medical issues - Educate patients and families on health maintenance, disease prevention, and lifestyle modifications - Collaborate with specialists, nurses, and support staff for coordinated, comprehensive care; participate in care coordination and case management - Maintain accurate medical records per hospital/clinic policies; participate in quality improvement initiatives and clinical audits - 2 years of primary care experience preferred; exceptional new graduates with outstanding training considered - Proficiency in EHR systems required; commitment to ongoing professional development - Competitive salary; health, dental, and vision insurance; 15 paid holidays; paid time off and continuing education opportunities
