Description: Facility is seeking a Long-Term Care Nurse Practitioner provide care to an aging patient population in long-term care settings - Conduct comprehensive assessments including medical history, physical examinations, and medication reviews - Develop and implement individualized care plans for long-term care residents - Monitor and manage chronic conditions in collaboration with physicians and specialists - Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care - Provide education and support to patients, families, and caregivers on disease management, preventive care, and wellness - Participate in regular team meetings and contribute to care process improvement - EMR: Epic - FNP or Adult-Gerontology Nurse Practitioner certification required - Active Maine Nurse Practitioner license required
