Description: Seeking a full-time Nurse Practitioner/Physician Assistant to join our interdisciplinary care team of physicians, advanced practice providers, nurses, social workers and other healthcare professionals. --Our mission is to provide high-quality care to frail and elderly members residing in participating skilled nursing and assisted living facilities. --Our providers work collaboratively with primary care physicians, specialists, facility staff and broader interdisciplinary care teams to support our members’ health, independence, comfort and quality of life. --We offer a collaborative work environment with supportive leadership and an opportunity to build meaningful relationships with members while making a direct impact on their care. --APP is primarily in the field actively managing an assigned caseload of members. --The role includes conducting initial and ongoing assessments, visiting members within nursing facilities regularly based on acuity and documenting care in the EHR according to company policy. --Along with proficient clinical skills, the candidate must be able to build and maintain collaborative, cooperative, productive relationships with all internal and external contacts including members and their representatives, nursing/adult home facility staff, physicians, the health plan and the interdisciplinary team. --Pay: $120,000.00 - $135,000.00 per year --$10,000 Sign-On Bonus --Additional Quality Bonus Incentives --401(k) matching --Dental insurance --Employee assistance program --Flexible schedule --Flexible spending account --Health insurance --Health savings account --Life insurance --Paid time off --Referral program --Required: Board Certification, NY license, CPR Certification, Completion of 35 continuing CME credits annually --Must have valid New York State driver's license and active automobile insurance that meets company standards
