Description: Seeking Nurse Practitioner to join Coumadin Clinic
--Shift: Primarily Mon - Fri / 8AM - 5PM
--Schedule: Part Time (0.6 FTE)
--The Nurse Practitioner provides professional health services through assessment, diagnosis, education and/or treatment for acute and chronic health problems along with preventive care and/or palliative/hospice.
--Per unit specific requirements, the Nurse Practitioner may function as a team member with other school, community and primary care providers.
--A Nurse Practitioner may manage the health center/department/clinic.
--Occupational health services to worker and clients.
--Urgent care services to the general population.
--Care to patients requiring psychiatric mental health services.
--Services in a school based health center serving enrolled students.
--Advanced practice nursing services through palliation and management of the terminal condition and development of treatment plans in collaboration with the IDG through administrative and symptom management for patients enrolled in hospice care.
--Advanced wound therapy to all ages of patients or have responsibility for delivering and administering comprehensive medical care across the age continuum to patients seeking pre-surgical assessment per Oregon State Board of Nursing scope described in Division 50 of the Oregon Nurse Practice Act, utilizing standards of advance practice.
--Anticoagulation therapy with warfarin (or Coumadin), integrating INR result monitoring, doing algorithms and assessment based on the Antithrombotic Therapy &Prevention of Thrombosis through American College of Chest Physicians (ACCP) Clinical Practice Guidelines.

--Preferred: FNP: Family Nurse Practitioner licensed by the Oregon State Board of Nursing
--Required:
- Graduation from an accredited Nurse Practitioner Program approved by the American Medical Association
- Certifications based on area of specialty and defined by our medical staff services
- NP: Nurse Practitioner licensed by Oregon State Board of Nursing (OSBN) AND RN: Registered Nurse licensed by Oregon State board of Nursing is required
- BLS: Basic Life Support from the American Heart Association (AHA)
- ACLS: Advanced Cardiac Life Support certification
