Description: Facility is seeking Primary Care phyisican to join team - Conduct thorough evaluations and assessments of patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests. - Develop and implement individualized treatment plans based on patient needs, medical history, and diagnostic results. - Provide preventive care services, including routine check-ups, immunizations, and health screenings. - Manage chronic conditions such as diabetes, hypertension, and asthma, and provide ongoing monitoring and treatment. - Address acute medical issues, including infections, injuries, and urgent care needs. - Educate patients and their families about health maintenance, disease prevention, and lifestyle modifications - Collaborate with other healthcare professionals, including specialists, nurses, and support staff, to provide coordinated and comprehensive care. - Communicate effectively with patients and their families about diagnoses, treatment options, and care plans. - Participate in care coordination and case management to ensure seamless patient care. - Maintain accurate and up-to-date medical records in accordance with hospital/clinic policies and regulatory requirements. - Participate in quality improvement initiatives and clinical audits to enhance patient care and operational efficiency. - Stay current with medical advancements, best practices, and guidelines in primary care. - Board-certified or board-eligible in Internal Medicine, Family Medicine, Med-Peds or a related specialty - 2 years of experience in primary care preferred.
