Description: Family Medicine sought for primary care position in a large multi-specialty, integrated practice health center. Experience with complex disabled or group home population a plus, but not required. Health Center is PCMH III certified with partnerships in ACO and local Safety Net Provider Organization. Physician responsibilities include full scope of medical care (ages 18 and up to include):
• Routine health maintenance
• Health promotion
• Disease prevention
• Chronic Disease management
• Clinical Outreach
Strong nursing support provided, as well as fully supported, nationally rated EMR with templated configurations for special programs and clinical quality indicator capture. Access to on-site physical therapy, occupational therapy, speech therapy, behavioral health, psychiatry, neurology and podiatry services provides exceptional, cutting edge focus for continuous care and integrated medical practice. Registered Nurse Case Management support for multi-faceted patient care needs, enhances physician leadership role in which care and clinical team services are fully coordinated. Call coverage is second call with first line of response at registered nursing level 24/7. Call is one in three monthly with average number of calls 2 – 5 per week. Currently the Health Center site is partially under renovation to provide a discrete medical suite with three new exam rooms, a waiting room, secure check-in and check-out capabilities and a nursing station. This expansion can support innovation through diverse use of the suite for private practice, specialty certification preferences or other physician practice interests. The Western New York area is on the cusp of health landscape transformations which will designate it in less than five years a as a major national research hub. Two large hospital systems, a strategic network of Community hospitals and multiple urgent care centers support health care needs in the region anchored through strong academic medicine ties to the State University of New York at Buffalo.
